Description
Engine speed below 1000 rpm, system voltage over 12v, Actuator Test off, TP Sensor signal under 1.16v, and the PCM did not detect any injector coil surge voltage for 3 ms after Injector 1-4 was turned off (a surge voltage of 2v higher than system voltage is expected).

Fuel injector 1-4 control circuit is open or groundedFuel injector 1-4 power circuit from the ASD relay is openFuel injector 1 has failed or PCM injector 1 driver has failed
